AND I'M JUST GOING TO TALK ABOUT OUR INVESTMENTS IN COMMUNITY HEALTH THROUGH OUR GRANTS TODAY. THIS EVENING YOU SAID, OKAY, GREAT. SO TEXAS HEALTH MISSION IS TO IMPROVE THE HEALTH OF THE PEOPLE AND THE COMMUNITIES THAT WE SERVE. AND SO WE RECOGNIZE IN ORDER TO DO THIS, WE HAVE TO GO BEYOND THE FOUR WALLS OF OUR HOSPITALS AND CLINICS, AND WE REALLY HAVE TO GO OUT INTO THE COMMUNITY TO IMPROVE COMMUNITY HEALTH. IN 2019, WE STARTED TEXAS HEALTH COMMUNITY IMPACT. AND SO WHAT WE DO IS WE INVEST IN COMMUNITY HEALTH AND WELL-BEING THROUGH GRANT MAKING AND STRATEGIC COLLABORATIONS. AND THIS IS OUR FORMULA, IF YOU WILL, FOR HOW WE IMPROVE COMMUNITY HEALTH. SO FIRST, WE'RE GUIDED BY DATA SUCH AS OUR COMMUNITY HEALTH NEEDS ASSESSMENT THAT IS DONE EVERY THREE YEARS. WE ALSO UTILIZE REGIONAL EXPERTS. SO THESE ARE COMMUNITY LEADERS WHO SERVE ON OUR LEADERSHIP COUNCILS. THEY INFORM OUR WORK AND SELECT OUR GRANTEES. AND THEN OF COURSE, WE SUPPORT COMMUNITY ORGANIZATIONS THROUGH OUR GRANTS. SINCE OUR INCEPTION IN 2019, WE'VE INVESTED A LITTLE OVER $18 MILLION ACROSS 49 GRANT PROGRAMS IN JOHNSON COUNTY. SPECIFICALLY, WE RECEIVED $1.2 MILLION. WE DO ALL OF THIS BECAUSE WE WANT TO MAKE A MEASURABLE CHANGE IN THE COMMUNITY, AND THAT QR CODE OR THAT URL AS WELL, TEXAS HEALTH.ORG FUTURES WILL SEND YOU TO OUR FIVE YEAR REPORT, WHERE YOU CAN LEARN MORE ABOUT THE GRANT PROGRAMS THAT WE FUNDED. AND HERE ARE SOME TESTIMONIES FROM PEOPLE WHO BENEFITED DIRECTLY FROM OUR PROGRAMS. OKAY, SO IN THE SOUTHERN REGION, WHICH CONSISTS OF ERATH AND JOHNSON COUNTY, WE'VE ADAPTED OUR APPROACH TO BETTER MEET THE NEEDS OF OUR COMMUNITY. AND I'M SAYING OUR BECAUSE I AM FROM JOHNSON COUNTY, I GREW UP IN CLEBURNE. I GRADUATED FROM VENUS HIGH SCHOOL. AND SO IN 2019, WE DID START WITH TRADITIONAL GRANTS. TRADITIONAL COMMUNITY HEALTH GRANTS, WHERE WE IMPLEMENTED PROGRAMS TO IMPROVE COMMUNITY HEALTH. AT THAT TIME, OUR H AND A GUIDED US TO TARGET FOOD SECURITY AND REDUCE DEPRESSION. AND WHILE THE GRANT MADE SOME PROGRESS, WE REALIZED THAT IN ORDER TO MAKE A LASTING IMPACT, THAT WE OUGHT TO INVEST IN ORGANIZATIONAL CAPACITY AND COMMUNITY COLLABORATION. AND THAT'S BECAUSE OUR NONPROFITS OFTEN HAVE THE HEARTS, BUT NOT THE RESOURCES TO DO MEANINGFUL WORK. AND SO WE INVESTED IN THAT FOR TWO GRANT CYCLES. AND IN OUR CURRENT GRANT CYCLE, 2025, 2026, WE TOOK A TWO PHASE APPROACH, AND I WILL DETAIL THAT LATER. BUT NOW I WANT TO TALK A LITTLE BIT ABOUT OUR NEXT GRANT CYCLE, WHICH IS GOING TO START IN 2027 AND END IN 2028. WE INTEND TO FOCUS MORE ON INNOVATION. I MENTIONED EARLIER THAT WE ARE DATA DRIVEN, SO I WANT TO JUST HIGHLIGHT OUR 2020 5CH IN A THAT WAS RELEASED EARLIER THIS YEAR. AS YOU CAN SEE HERE, THESE ARE THE TOP FOUR IDENTIFIED NEEDS, THE NON-MEDICAL DRIVERS OF HEALTH THAT IMPACT OUR DAILY LIVES FAR MORE THAN CLINICAL CARE ALONE. SO YOU CAN TAKE A LOOK THERE. I CAN TELL YOU TODAY THAT THE SOUTHERN REGION AIMS TO TARGET HEALTHCARE ACCESS AND CONNECTEDNESS FOR THE NEXT GRANT CYCLE. I ALSO WANT TO HIGHLIGHT FOR THE 2020 5CHNA THAT 76009 OR ALVARADO WAS ADDED TO OUR SERVICE AREAS. IT WAS PREVIOUSLY NOT IN OUR SERVICE AREAS. SO THAT'S ONE OF THE REASONS WHY I'M HERE TODAY, IS TO LET YOU GUYS KNOW THAT YOU GUYS NOW ARE OPEN TO SOME OF OUR GRANT OPPORTUNITIES. AND SPEAKING OF GRANT OPPORTUNITIES, OUR RFP, IT MIGHT BE THE NEXT SLIDE. LET ME SEE. OH NO IT'S NOT OKAY. I MIGHT BE MISSING QUITE A FEW SLIDES. LET'S SEE.

OKAY, SO OUR CURRENT GRANT CYCLE REQUEST FOR PROPOSAL WAS OPENED ON APRIL 13TH, AND IT WILL CLOSE ON 1159 ON MAY 29TH. AND SO OUR REQUEST FOR PROPOSALS IS AGAIN LOOKING AT ACCESS AND CONNECTEDNESS FOR CAREGIVERS, SPECIFICALLY IN ALVARADO, KEEN, CLEBURNE AND RIO VISTA. AND THEN SO IF THERE ARE ANY COMMUNITY ORGANIZATIONS THAT ARE INTERESTED IN APPLYING FOR OUR GRANTS, HERE ARE OUR FOUR KEY GRANT REQUIREMENTS. AGAIN, INNOVATION IS A FOCUS

AND THAT'S GOING TO BE POSTED ON OUR WEBSITE SO THAT YOU GUYS CAN LEARN MORE ABOUT WHAT WE DEFINE AS INNOVATION AND HOW TO BE INNOVATIVE IN YOUR GRANT PROPOSALS. PROPOSALS MUST ALSO ALIGN WITH OUR STRATEGIC PLAN, WHICH IS DETAILED IN OUR RFP, WHICH IS POSTED AT TEXAS HEALTH.ORG SLASH T C I DASH RFP. SO WE FUNDED THEM TO WORK TOGETHER TO CREATE A GRANT PROPOSAL RATHER THAN IMPLEMENTED. AND SO THEN OUT OF THOSE THREE COLLABORATIVES, OUR LEADERSHIP COUNCIL SELECTED ONE COMMUNITY COLLABORATIVE TO ACTUALLY IMPLEMENT THEIR PROGRAM. AND SO REACH COUNCIL LED THE TOGETHER WE GROW COLLABORATIVE AND ARE CURRENTLY IMPLEMENTING THE KEYS TO SUCCESS PROGRAM, WHICH SHE CAN TALK ABOUT. THANK YOU. ALL RIGHT. BUT JUST TO TALK A LITTLE BIT ABOUT IT. SO LIKE AARON SAID, IT WAS A TWO PHASED APPROACH.

WE GOT SIX MONTHS TO DIVE INTO THE COMMUNITY AND SEE WHAT WAS GOING ON. AND IN THAT SIX MONTHS, ONE OF THE STATISTICS THAT REALLY STUCK TO ME IS THE STATISTIC OF DISCONNECTED YOUTH IN OUR COMMUNITIES, 18% OF 16 TO 19 YEAR OLDS IN JOHNSON COUNTY ARE NOT GOING TO SCHOOL, AND THEY'RE NOT EMPLOYED. THE STATE AVERAGE IS 9%. SO WE ARE ALMOST DOUBLE THE STATE AVERAGE IN OUR COUNTY. AND, YOU KNOW, THINK ABOUT THE POPULATION OF OUR COUNTY VERSUS THE STATE AND ALL OF THAT. WE HAVE A HIGH DISCONNECTED YOUTH RATE HERE. AND WHY, WHAT GAPS ARE THERE IN OUR COMMUNITIES THAT ARE CAUSING THESE YOUTH TO JUST GET DISCONNECTED AT AN EARLY AGE AND NEVER RECONNECT? SO WE STARTED LOOKING AND WE CREATED SEEDS TO SUCCESS. SO THIS IS A MENTORSHIP PROGRAM FOR YOUTH AGES 15 TO 25 THAT ALLOW THEM TO EXPLORE CAREER PATHS OR EDUCATION PATHS AT NO COST TO THEM. IT'S A COMPLETELY FREE PROGRAM. WE OFFER MENTORSHIP. SO ONE OF THE CONSISTENT THEMES COMING OUT OF THESE CONVERSATIONS IS THAT INFRASTRUCTURE AND TRANSPORTATION COORDINATION ARE CRITICAL TO MAKING OUR SITE'S DEVELOPMENT READY AND COMPETITIVE.
